Self-Referral Path
- Self-Referral Form: If you would like more information about The Recovery Community Centre, please fill in the Self-Referral Form.
- Follow up call or Welcome Tour: Once you have filled out the Self-Referral Form, you will be called or emailed by the Program Coordinator to set up a tour time and date. This tour will be facilitated by a Peer Coach and can be done individually or in a group, depending on how many people are interested in receiving a tour.
Community Referral Path
- Community Referral Form: A representative from the referral organization will work collaboratively with the person being referred to fill out a Community Referral Form.
- Program Coordinator Contact: The Program Coordinator will contact the person (email or phone) to set up a potential tour date and time or answer any questions the individual has.
- Tour: A tour of the facility will be given by a Peer Coach and can be done individually or in a group, depending on how many people are interested in receiving a tour.
Walk-In
- Walk-In & Fill in Form: When a person walks in through the door seeking information, they will be greeted by a Peer Coach and/or Recovery Community Centre Staff who will ask them if they were referred by another organization. If they were, the prospective member will fill out the Community Referral Form and if not, they will fill in the Self-Referral Form.
- Questions and/or Tour: If the prospective member is interested in a tour, it will be given by a Peer Coach and can be done individually or in a group, depending on how many people are interested in receiving a tour. If the prospective member would just like to learn more and ask questions without a tour, this can be done as well.
